Submit a registration form
We ask that, if at all possible, we receive your registration and payment at least 2 weeks prior to the class date. To register for classes, please fill out, sign, and submit our web-based registration form.
Alternatively, we still have a PDF registration form that you can print and fax to 770-414-5661 or scan and email to email@example.com. You can download it, or request that we fax you the form by emailing us or giving us a call at 770-414-5007. No cover sheet is required. If you have any problems with the fax line, our back up fax number is 312-896-9094.
We must have a signed copy of your registration form with payment before we can confirm your seat.
Payment can be made with a card such as Visa, American Express, MasterCard, or Discover. If you put credit card details on the registration form, then no further action is required of you.
We can also accept a mailed check. However, we will need to receive it prior to the start date of the class. If the class date is less than 2 weeks away, please also fax or email a scanned copy of the check so that we can reserve a seat for you.
Within 24-48 hours of receiving your signed form with payment, we will send you confirmation of your seat in our class. We look forward to having you.
