If at all possible, registrations and payment should be received at least 2 weeks prior to class date.

This registration form is an Acrobat PDF file that you can fill out on your computer using Acrobat Reader version 4.0 or later.

We need a signed paper copy faxed to us in order to process the registration.

If you would prefer us to fax you a copy of the registration form, please email us or call us at 770-414-5007.

Print out the form, sign it, and fax it to us at
312-896-9094.

 

We must have a signed copy of your registration form with a credit card payment to confirm your seat. We accept Visa, MasterCard, American Express, and Discover.

If you are mailing a check to pay for your class, please fax us a copy of the check with your form.

Your seat is not held or confirmed until payment is received.

Within 48 hours (typically within 24 hours), you will receive an email acknowledgement of your registration.
