Submit an enrollment form
To enroll in classes, please fill out, sign, and submit our web-based enrollment form.
If at all possible, Ledet training asks that we receive your enrollment and payment at least 2 weeks prior to the class date.
Alternatively, we still have a PDF enrollment form that you can print and fax to 770-414-5661 or scan and email to email@example.com. You can download the form, or request that we fax you the form by emailing us or giving us a call at 770-414-5007. No cover sheet is required if faxing.
We must have a signed copy of your enrollment form with payment before we can confirm your seat.
Payment can be made with a card such as Visa, American Express, MasterCard, or Discover. If you put credit card details on the registration form, then no further action is required of you.

We can also accept a mailed check. However, we will need to receive the check prior to the start date of the class. If the class date is less than 2 weeks away, please also fax or email firstname.lastname@example.org with a scanned copy of the check so that we can reserve a seat for you.
Within 24-48 hours of receiving your signed enrollment form with payment, Ledet Training will send you confirmation of your seat in our class.
