This engagement form forms the basis of your class quotation request. By filling it out, we can best understand your training needs and scheduling requirements. It will become your final engagement agreement once you've agreed to our proposal and engaged us to deliver your training.
Final signed engagement agreement with payment should be received at least 4 weeks prior to class date to allow us to arrange for travel and instructor scheduling.
The form is a pdf you can fill out on your computer, however if you would prefer we fax you the form, email us or give us a call at 770-414-5007. You can also scan and email us your filled out engagement form at registrations@ledet.com.

We must have a signed copy of your engagement form and payment before we can confirm arrangements for training at your site. No dates can be secured in advance of payment.
Payment can be made with a card such as Visa, American Express, MasterCard, or Discover. We can also accept a mailed check. If you are mailing a check for payment, please attach a scanned copy of your check to the engagement form.
Confirm engagement and arrange logistics
Within 24 hours of receiving your signed engagement form with payment, we will contact you to confirm details and arrange logistics. We look forward to serving you.
